Hello!

I am writing to invite you to participate in a survey I am conducting about your personal experience of video hosting services. Anyone over the age of 18 who uploads video to the web is welcome to contribute. The link below leads to a survey that will take less than 10 minutes to complete. Your participation is entirely voluntary and anonymous.

Digital video practitioners are needed for a study concerning online video habits. The survey will take approximately ten minutes to complete. It is entirely voluntary and participants may quit at any time. No identifying information is collected. Thank you!

INFORMATION/FACTS SHEET FOR NON-MEDICAL RESEARCH

SHORT STUDY TITLE: CONTINUANCE AND VIDEO HOSTING SERVICES

You are being asked to participate in a research study conducted by Kevin Driscoll, a doctoral student at the Annenberg School for Communication and Journalism at the University of Southern California. To participate in this study, you must be at least 18 years old. Please note that your participation is entirely voluntary. You may also decide to opt out at any time without consequence. Initiating the online survey will be taken as constituting consent to participate in the research project.

PURPOSE OF THE STUDY

By participating in this study you will be helping to further research that evaluates the online video environment for better understanding of the practitioner experience.

PARTICIPANT INVOLVEMENT

You will be asked to answer a few questions about your digital video practices. Afterwards you will be asked to fill out a short survey about your recent experiences with your preferred video hosting service. This survey will take approximately 10 minutes to complete.

CONFIDENTIALITY AND POTENTIAL RISKS

No identifiable information will be obtained in connection with this study, including your name or address. No risks are anticipated involving your participation.

Debriefing

The purpose of this research is to better understand the the practitioner experience of the affordances and constraints of the online video environment.

Confidentiality

As a reminder, your name, address or other information that may identify you will not be collected during this research study. Only members of the research team will have access to the data associated with this study. The data will be stored in the investigator’s office in a password protected computer.

The data will be stored for one year after the study has been completed and then destroyed.

If the results of the research are published or discussed in conferences, there will be no information that will be included that may reveal your identity.
